“Chemical peeling simply strips away the skin.”
“Isn’t chemical peeling unsuitable for sensitive skin?”
If these are your impressions of chemical peeling, you may be surprised by how much modern peeling formulations have evolved.
In this article, I will introduce one of my favorite treatments, Milano RePeel (BioRePeel), and explain the medical principles behind its effects on the skin.
目次
Benefits of Milano RePeel
Improvement in Acne Scars and Hyperpigmentation
Milano RePeel promotes epidermal turnover, helping shed accumulated dead skin cells containing melanin. This process encourages the renewal of the skin’s outer layer and may improve uneven pigmentation.
The acids contained in Milano RePeel help loosen the connections between skin cells, including structures known as desmosomes.
As a result, older skin cells containing melanin can be shed through exfoliation, sometimes appearing as mild, visible peeling.
By gradually removing superficial pigmented skin cells, repeated treatments may help promote a more even, clearer, and brighter-looking complexion.
Improvement in Enlarged Pores and Clogged Pores
“Why can Milano RePeel be effective for clogged pores compared with other peeling treatments?”
The key lies in salicylic acid, a beta-hydroxy acid (BHA), which is oil-soluble.
Pores contain sebum, an oily substance produced by the skin.
Alpha-hydroxy acids (AHAs), commonly known as fruit acids, are water-soluble and primarily work on the skin’s surface.
In contrast, oil-soluble BHA can act within sebum-rich pores, helping loosen accumulated dead skin cells and oily debris.
By reducing the buildup of sebum and keratin plugs that contribute to clogged pores, BHA may help improve their appearance.
Combined with the skin-renewing effects of TCA, Milano RePeel may help make enlarged pores appear smaller and improve overall skin texture.
Improvement in Fine Lines and Loss of Elasticity Caused by Photoaging
“How can a chemical peel improve skin firmness from within?”
One of the key characteristics of TCA (trichloroacetic acid) is its ability to stimulate the skin’s wound-healing response, a natural repair and regeneration process.
When TCA produces a controlled chemical effect within the skin, it may activate repair mechanisms.
This process can stimulate fibroblasts, the cells responsible for producing collagen and other components that support skin structure.
One particularly interesting type is type III collagen, sometimes referred to as “baby collagen” because it is abundant in younger skin and contributes to its flexibility.
By supporting collagen remodeling and the skin’s structural components, the treatment may help improve firmness, elasticity, and the appearance of superficial fine lines.
Why Is Intentional Exfoliation Important?
Massage Peel (PRX-T33) uses a formulation containing hydrogen peroxide and TCA, designed to stimulate the skin while minimizing visible epidermal peeling.
This makes it an appealing option for patients who wish to minimize downtime.
However, depressed acne scars and rough, uneven skin texture involve structural changes in the skin itself.
Addressing these concerns may require more than improving dermal firmness alone. Treatments that promote controlled exfoliation and skin remodeling may also be considered.
Milano RePeel is designed to produce epidermal exfoliation, which may involve visible peeling.
Although this can involve several days of downtime, the exfoliation process helps remove accumulated surface skin cells and supports renewal.
For patients seeking to improve skin texture and superficial irregularities, Milano RePeel may be a suitable treatment option.
Key Feature: Advanced Biphasic Technology
Two-Phase Formulation
Milano RePeel consists of two distinct phases: a blue hydrophilic phase and a yellow lipophilic phase.
・Lipophilic Phase (Yellow)
This phase helps protect the acids from oxidation and maintain the stability of the formulation.
When applied to the skin, it forms a protective film that helps reduce moisture loss while supporting the delivery of active ingredients.
・Hydrophilic Phase (Blue)
This phase contains TCA (trichloroacetic acid), AHAs, BHAs, vitamins, amino acids, and other active ingredients.
These components provide exfoliating and skin-stimulating effects.
When the two phases are mixed, the formulation is designed to deliver the benefits of a relatively high acid concentration while helping minimize irritation and discomfort.
35% TCA: Targeting Skin Remodeling
Milano RePeel contains TCA (trichloroacetic acid) at different concentrations depending on the treatment area.
The facial formulation contains 35% TCA, while the body formulation contains 50% TCA.
Biostimulation
TCA stimulates controlled skin renewal and may activate fibroblasts within the dermis.
・Collagen production: Supports the formation of collagen, which contributes to skin firmness.
・Elastin production: May support the remodeling of components involved in skin elasticity.
Rather than acting solely on the skin’s surface, Milano RePeel aims to support skin remodeling and improve the appearance of fine lines and uneven texture.
Exfoliating Action
At the same time, the peeling ingredients loosen the bonds between accumulated dead skin cells.
This helps remove superficial pigmented cells and debris that can contribute to clogged pores.
The resulting exfoliation encourages the renewal of the outer skin layer.
This is why visible skin peeling may occur during the days following treatment.
More Than Acids: A Nutrient-Rich Formulation for Skin Renewal
One characteristic that distinguishes Milano RePeel from conventional chemical peels is the combination of exfoliating acids with additional skin-conditioning ingredients.
Type III Collagen-Supporting Ingredients (GABA, Arginine, and Glycine)
The formulation contains amino acids and other ingredients associated with skin-conditioning and repair processes.
These ingredients are intended to support the skin’s repair and renewal following controlled exfoliation.
GABA is also included in the formulation and is associated with relaxation-related biological processes. Its direct effect on facial wrinkles when applied topically has not been firmly established.
Vitamins C and B2
Vitamins C and B2 support normal skin function, while vitamin C is also known for its antioxidant properties.
Milano RePeel combines exfoliation with skin-conditioning ingredients to support the appearance of smoother, healthier-looking skin following recovery.
Downtime After Milano RePeel
With Milano RePeel, mild skin peeling may occur approximately 3–7 days after treatment.
This is a commonly expected response to the exfoliating action of the treatment and is associated with the renewal of superficial skin cells.
Advice from an Aesthetic Nurse ✨
During this period, forcibly removing peeling skin can increase the risk of post-inflammatory hyperpigmentation (PIH).
The newly exposed skin may be particularly sensitive, making proper aftercare essential.
We recommend thorough moisturization with suitable skincare products containing ingredients such as ceramides, as well as strict sun protection.
Maintaining a healthy environment for skin recovery is important for achieving the best possible results.
👑 When applying toner or moisturizer, gentle repeated application can be more beneficial than applying a large amount all at once. 👑

Conclusion
In aesthetic dermatology, Milano RePeel, containing 35% TCA in its facial formulation, is more than a conventional exfoliating treatment.
It is designed to promote skin renewal and remodeling through the combined action of multiple peeling ingredients.
Its main features include exfoliation using salicylic acid (BHA) and various AHAs, together with the skin-stimulating effects of TCA.
These mechanisms may help improve superficial skin texture, uneven pigmentation, and the appearance of fine lines.
Milano RePeel may be suitable for people who want to:
🌟 Improve the appearance of acne scars and hyperpigmentation
🌟 Reduce the appearance of enlarged pores and clogged pores
🌟 Address fine lines and loss of skin elasticity associated with photoaging
If you are comfortable with several days of potential downtime, Milano RePeel may be a valuable option for improving overall skin texture and appearance.

Frequently Asked Questions (FAQ)
Q1: I’m worried about pain during treatment because the formulation contains 35% TCA. 💦
A: Many patients describe the sensation as warmth or mild stinging, rather than severe pain.
Because the treatment uses a relatively high concentration of acid, you may experience a temporary warming sensation or tingling while the solution is applied.
However, its specialized two-phase formulation is designed to reduce irritation, and many patients find the discomfort tolerable.
The sensation generally subsides after the product is removed.
Q2: When can I wear makeup after Milano RePeel?
A: Makeup can generally be applied the following day, but please be aware that peeling may begin several days later.
Immediately after treatment, the skin is particularly delicate. For this reason, we ask patients to avoid makeup on the day of the procedure.
You can usually resume your normal makeup routine the next day.
However, when mild peeling begins around the third day, foundation may not apply as smoothly as usual.
We recommend using a moisturizing makeup base and, if needed, a face mask to help conceal visible flaking.
Q3: Can I see results after just one treatment?
A: Some patients notice smoother skin texture after a single session. However, for more noticeable improvements, a series of 4–6 treatments may be recommended.
Even one session can help remove accumulated dead skin cells, making the skin feel smoother and appear brighter.
However, concerns such as uneven texture associated with acne scarring and reduced skin firmness may require multiple treatments.
Depending on your skin condition and treatment goals, our clinic may recommend an initial series of 4–6 sessions at intervals of approximately 2–3 weeks.
Individual results vary, and the appropriate number of sessions will be determined during consultation.
Q4: Will I need to take time off work because of the downtime?
A: In most cases, taking time off work is not necessary.
Although skin peeling may occur, it generally does not involve large sheets of skin peeling away like after a severe sunburn.
Instead, the skin may shed in small, thin flakes, similar to tiny eraser shavings.
The peeling is often mild enough to be relatively unnoticeable when wearing a face mask, allowing many patients to continue their usual daily activities.
